Please bring your insurance card to the visit.Many insurance plans require preauthorization for specialist appointments. If your plan requires preauthorization, it is your responsibility to contact the primary care physician prior to the appointment to request preauthorization. It is also your responsibility to follow up prior to the appointment to make sure the authorization has been issued. If you do not have proper preauthorization at the time of the visit, you will be expected to pay the full fee for the visit at the time of the appointment or reschedule the appointment. Many insurance companies do not cover routine eye care or vision care. Check with your insurance company prior to the appointment to see if routine and vision care is covered.
